To find a network dentist, visit principal.com above or call (800) 247-4695. Using a network dentist may lower your costs, as they agree to reduced fees and won’t bill you the difference, with access to over 117,000 providers nationwide.
- Copies of marriage licenses and birth certificates are required for dependents’ coverage.
- Member must be actively working as a 1099 contractor to be eligible (unless you are part of a group plan).
- In-network only coverage except for emergency. See SBC for details.
- There is no access to BCBS online portal. You will be sent your Explanation of Benefits (EOB) in the mail directly to your address.
- The plan is a one-year minimum contract. Month to month thereafter.
- All individual enrollments need to be completed by the 15th of the month at 5 pm CST. If the 15th falls on the weekend, enrollments must be completed by the following Monday by 5 pm CST. (Completed enrollments include payment information, marriage licenses and birth certificates)
