  • Copies of marriage licenses and birth certificates are required for dependents’ coverage.
  • Member must be actively working as a 1099 contractor to be eligible (unless you are part of a group plan).
  • In-network only coverage except for emergency. See SBC for details.
  • There is no access to BCBS online portal. You will be sent your Explanation of Benefits (EOB) in the mail directly to your address.
  • The plan is a one-year minimum contract. Month to month thereafter.
  • All individual enrollments need to be completed by the 15th of the month at 5 pm CST. If the 15th falls on the weekend, enrollments must be completed by the following Monday by 5 pm CST. (Completed enrollments include payment information, marriage licenses and birth certificates)
