Visit vsp.com to find VSP doctors near you or check if your current eye care professional is in the VSP network. Select the “Choice” doctor network to see the VSP providers covered by your plan.
- Copies of marriage licenses and birth certificates are required for dependents’ coverage.
- Member must be actively working as a 1099 contractor to be eligible (unless you are part of a group plan).
- In-network only coverage except for emergency. See SBC for details.
- There is no access to BCBS online portal. You will be sent your Explanation of Benefits (EOB) in the mail directly to your address.
- The plan is a one-year minimum contract. Month to month thereafter.
- All individual enrollments need to be completed by the 15th of the month at 5 pm CST. If the 15th falls on the weekend, enrollments must be completed by the following Monday by 5 pm CST. (Completed enrollments include payment information, marriage licenses and birth certificates)
